Making sure our pet family is appropriately taken care of when sick or injured is an important aspect of owning a pet. We ask that you provide information on your current and/or past vet so that we can ascertain if your pets have continued to receive necessary care in the past. If you do not currently have pets and thus do not currently have a vet, please use the short-answer form at the end of the section to let us know what you will be looking for in a veterinarian going forward.
